Pneumococcal disease refers to any illness affect various part of body caused by bacterium Streptococcus pneumoniae (pneumococcal bacteria). Bacteria can cause many types of illnesses, including pneumonia, which is an infection of the lungs. 

Symptoms of Pneumococcal disease:

* Sinus infections
* Pneumonia

Pneumococcal vaccine in Thailand:
- PCV13 (Prevnar 13) contains 13 polysaccharide serotypes of S pneumonia.
The Prevnar 13 decreased incidence of invasive pneumococcal disease (IPD), reduced transmission across all age groups and unvaccinated populations.
- PPSV 23 (Pneumovax 23) is a active immunization for the prevention of pneumococcal disease, contain 23 polysaccharide serotypes of S pneumonia.

Recommended for:
For children get Prevnar 13 at 2 months, 4 months, 6 months, and 12-15 months of age

Adults ages 19-64 years
Adults aged 19 to 64 years with chronic diseases (Diabetes mellitus, congestive heart failure, cardiomyopathy, chronic liver disease, chronic lung disease, chronic obstructive pulmonary disease, asthma, chronic renal failure, cigarette smoking) and  immunocompetent person (CSF leak, HIV, Hodgkin’s disease, iatrogenic immunosuppression, leukemia, lymphoma, multiple myeloma, sickle cell disease, hemoglobinopathy, organ transplant) that put high risk for IPD (Invasive streptococcal disease) should receive one dose of PCV13.
- Chronic disease: one dose of Prevnar 13 should be given if the you has never received a pneumococcal vaccine and followed by a dose of Pneumovax 23 at least 1 year after the Prevnar 13
- Immunocompetent person: 1 dose of Prevnar 13 should be given and followed by a dose of Pneumovax 23 at least 8 week after the Prevnar 13 one dose of Pneumovax >5 years after the last pneumococcal dose

Adults aged > 65 years
- One dose of Prevnar 13 should be given if the you has never received a pneumococcal vaccine and followed by a dose of Pneumovax 23 at least 1 year after the Prevnar 13
- If you have a cochlear implant, CSF leak, or an immunocompromised state 1 dose of Prevnar 13 should be given and followed by a dose of Pneumovax 23 at least 8 week after the Prevnar 13 one dose of Pneumovax >5 years after the last pneumococcal dose is recommended.

- Muscle pain
- Weakness

 

MMR (measles, mumps and rubella) VACCINE
MMR vaccine can prevent as below details:
- Measles causes of fever, cough, runny nose, and red, watery eyes, followed by a rash that covers the whole body and can lead to seizures, ear infections, diarrhea, and pneumonia.
- Mumps causes of fever, headache, muscle aches, tiredness, loss of appetite, and swollen and tender salivary glands under the ears.
- Rubella causes of fever, sore throat, rash, headache, and eye irritation.

MMR vaccine is recommended for:
- Children
- Adults
- Travelers
- Healthcare professionals
- Pregnant before 28 weeks gestation

Administration
- Children should get first dose at 12 through 15 months of age, and the second dose at 4 through 6 years of age.
- Others should get 2 doses interval 28 days

MMR side effects

Tdap(Diphtheria, Tetanus, Pertussis) vaccine

Tdap vaccine can prevent as below details:
- Diphtheria is a respiratory disease that can cause breath problems. It's highly contagious and is spread by cough and sneezing.
- Tetanus is caused by a bacterium often found in soil. Enters the body will releases a toxin that attacks the nervous system, causing muscle spasm and death.
- Pertussis is caused by a bacterium, causes coughing spasms so severe that in infants it can lead to pneumonia and death.

Administration
- Children, should receive at age 2 months, 4 months, 6 months, 15 to 18 months, and 4 to 6 years
- Pregnant women, should have a dose during 27 to 36 weeks gestation

Tdap is recommended for:
- Children at ≥ 13 years who have never received Tdap
- Vaccine status is unknown
- Pregnant women during each pregnancy
- Postpartum women who have never received Tdap

Herpes Zoster (Shingles) vaccine

Shingles vaccine is recommended for:
- Have already had shingles
- Have been vaccinated against shingles with Zostavax
- Are not sure if you have chickenpox
- 50 years and older
- 19 years and older who have a weakened immune system
- Pregnant women

Administration
- Immunocompetence adults 50 years and older and adults > 19 years get 2 doses, 2-6 months apart (0,2-6months)

Varicella vaccine
Varicella also called “chickenpox” is a live attenuated (contains a small amount of weakened chickenpox-causing virus). Varicella vaccines stimulates your immune system to produce antibodies that will help protect against chickenpox.

Varicella vaccine is recommended for:
* First dose of chickenpox vaccine at age 12 months to 15 months and a second dose at age 4 to 6 years
* If ages 13 and older, who have never been vaccinated or never had chickenpox should get 2 doses 4 to 8 weeks apart

Who is recommended to get varicella vaccine
- All children, beginning at age 12 months.
- Adults who have close contact with people at high risk disease (immunocompromised people) and who have high risk for exposure or transmission (teachers, child care, international travelers, military personnel, etc)
